Umpqua Health is a Coordinated Care Organization (CCO) in Roseburg, Oregon that serves over 30,000 OHP members within Douglas County, offering benefits, programs and outreach for those in need. Currently we are seeking a System of Care & Wrap Around Coordinator, to join our Behavioral Health team.

• Full-time community based and interactive position.

• $54,000-$75,000 Annual Salary DOE.

You can make a difference in Douglas County, working as a System of Care and Wrap around Coordinator with Umpqua Health. We are looking for candidates who have behavioral health experience, and bachelors degree in a related field to work as a leader with the Douglas System of Care (SOC). The SOC is a collaborative network of community services, for the health of youth ages 0-25. The SOC works to improve services and support families, using care values and principles to meet the contractual requirements. The candidate must be organized, and able to collaborate and bring people together building relationships within the community.
