Job Summary | Patient Care – 50% – Facilitate clinical navigation to prevent and address barriers to care including emotional, financial, practical, and physical concerns to patients at Markey Cancer Center ambulatory clinics and guide/refer to appropriate internal and external resources. Utilize social determinants of health to maximize outcomes towards health and psychosocial well-being. Facilitate education to patients and families regarding emotional and practical problems, services, and support. Participate in interdisciplinary care meetings and outreach activities (tumor boards, team meetings, committees) to foster patient and clinical team education regarding psychosocial support. Provide direct counseling and evidenced-based brief therapeutic interventions to patients and families through assessment and referring to internal and external oncology and community resources. Thoroughly address any potential barriers to treatment not limited to lodging, transportation, medications, advance care planning, suicide assessment, end-of-life care discussions, financial and insurance assistance, protection and permanency issues, education, and referrals. Oncology Distress Screening – 30% – Responding to Markey Cancer Center distress screening as per accrediting guidelines. Perform distress screening contacts either in person or by telephone and provide education regarding services to address barriers to care. Assist patients in a prompt and courteous fashion following UKHC BEST initiatives. Complete distress screening and document distress within the EMR. Communicate with MCC clinical team to assist patients and families with needs and follow patients through the continuum of care. Education – 15% – Facilitate cancer-related support groups utilizing psychoeducation and evidence-based group interventions. Provide internship opportunities for graduate students; Assist with orientation of new employees within the department and in other departments; present information learned at workshops or conferences to colleagues formally and informally; Participate in research and analysis of data as appropriate; provide education on appropriateness and availability of goods and services to benefit patients, families, and enterprise; Serves as a resource to other professionals by providing training and education about psych-oncology services. Professional Development – 5% – Engage in local and national learning and education opportunities as well as national professional organizations specific to specialty populations. Completes appropriate continuing education for maintenance of social work licensure and/or certification within the specialty. Seeks CEU/learning opportunities to continue to provide evidence-based interventions as well as improve the quality of social work care for patients and families. Participate in LCSW supervision at the discretion of the Psych-Oncology Program Director. Complete all required UK HealthCare required programs and educational requirements necessary for employment, including but not limited to web-based learning modules. This position has been identified as supporting UK HealthCare, which receives funding from the Centers for Medicare and Medicaid Services (CMS). Therefore, all individuals hired into this position must comply with the federal CMS mandate issued in March of 2022. This mandate requires all new hires to provide proof of COVID-19 vaccination or obtain an approved medical or religious exemption as a condition of employment. Start of employment cannot occur until two weeks after receiving the full COVID-19 vaccination series or upon obtaining an approved exemption. Only vaccines approved by the World Health Organization can be accepted. |
