What is a Case Manager in the Assertive Outreach program?

Case Managers in Vail Place's Assertive Outreach Program provide intensive outreach to people with serious mental illness with complex needs who need support and services to maintain safety and stability in the community. Individuals may be facing hospitalization, homelessness, substance use disorders and/or chronic medical conditions. This program addresses their many needs in ensuring continuity of care, referrals to needed resources, care coordination, monitoring, discharge planning/transitions of care, housing support, and intensive goal planning.

Our innovative, collaborative service model aims to connect these individuals to vital resources, assist them to get the supports they need, and connect them with long-term services. We are seeking skilled case managers or service coordinators who have a passion for addressing complex needs and experience using a person-centered approach to engage clients in effective planning and problem-solving.

This position is temporarily remote, but will return to an in-person service model as soon as COVID-19 protocols allow. Applicants will be required to perform in-person work in the Twin Cities on a regular basis.

Essential Duties and Responsibilities:?

The following duties are normal for this position. These duties are not to be construed as exclusive or all-inclusive.?

  • Provide intensive outreach to locate and connect with referred individuals.?
  • Engage with a variety of individuals and build genuine, therapeutic, trusting relationships with others, being sensitive to peoples past experiences and trauma.??
  • Work with a small inter-disciplinary team and contribute to a positive team atmosphere, including sharing clinical thoughts and ideas during weekly team meetings.??
  • Complete required intake paperwork and provide referrals and case coordination for individuals receiving services, including mental health, medical, financial, social, vocational, housing, and legal services.?
  • Complete required documentation, including assessments, crisis plans, releases, and Individual Goal Plans on schedule, per performance standards.?
  • Provide crisis assistance and intervention, including use of community crisis services.??
  • Maintain and develop positive and professional ongoing relationships with contracted Managed Care Coordinators, landlords, family members, employers, county & community agencies and other service providers.??
  • Complete case notes on all pertinent services provided to individuals receiving services within timelines.?
  • See individuals assigned to Assertive Outreach team based on current needs; prioritize the needs of all individuals receiving services and increase or decrease level of support when needed.?
  • Complete any statistics, reports or other data collection as required, per performance standards.?
  • Attend Team Meetings and regular meetings with contracted Managed Care Team members and one-on-one Supervision meetings as required.?
  • Return calls to individuals receiving services, providers, and others within 24 hours.

Minimum Qualifications

  • BA or BS in human services or related area, or BSW. BSW must have valid and current Social Work licensure.
  • Minimum two years experience in mental health or human services.
  • Must have and maintain a valid Minnesota Drivers License, maintain automobile coverage and have access to an automobile.
  • Must be willing and able to perform essential job functions through in-person, face to face contact as required, in accordance with health and safety guidelines.
