JOB SUMMARY:

To promote the mission of PATH to help individuals achieve a more independent and fulfilling life by being an effective team member. The BH/ID Specialized Residential Service provides direct care including support, structure, assistance, skill training, and therapeutic interventions, for service participants, in collaboration with BH/ID Integrated Clinical Services team. Services are provided in the home and community.

SPECIFIC DUTIES:

1. Provide supervision of residents at all times in accordance with their integrated individual support plan.

2. Maintain a supportive, therapeutic relationship with each service participant.

3. Be aware of each service participant's individual service plan, history, medications, current needs and any special accommodations required by the participant

4. Act as a role model at all times, maintaining appropriate boundaries and demonstrating effective communication skills

5. Maintain clear and consistent expectations of each service participant while maintaining a safe, supportive environment

6. Implement, on a consistent basis, direct service interventions as defined in the individual's service plans

7. Oversee, teach skills and actively assist in the completion of household chores ensuring the home is neat and clean at all times

8. Oversee, teach skills and actively assist in the completion of personal hygiene and other activities of daily living as needed by the participant

9. Assist service participants in developing and practicing skills defined on their individual service plan. This may include accompanying residents to appointments, social activities and/or job opportunities, and other activities of daily living.

10. Maintain open and ongoing collaboration and communication with all team members such as: residential direct care staff, clinicians, behavior specialists, psychiatrist, nurse and peer specialists

11. Maintain ongoing communication with each service participant's family/significant others utilizing diplomacy and tact and creating a welcoming environment

12. Ensure any family concerns are shared immediately with a supervisor

13. Participate in household shopping, as assigned

14. Coordinate, facilitate and engage in regular social activities with the service participants

15. Transport and accompany residents to appointments and activities, as assigned

16. Provide program coverage/shift coverage as assigned

17. Upon successful completion of training, monitor service participants taking their prescribed medications

18. Upon successful completion of training and in accordance with PATH policies and procedures, implement approved manual restrictive procedures if absolutely necessary to prevent a resident from harming himself or others

19. Provide crisis intervention in a calm therapeutic manner when indicated

20. Follow the When to Call a Supervisor' protocol which includes immediately contacting a supervisor, or the on-call supervisor, regarding any incidents, issues or concerns

21. Participate in scheduled meetings, as appropriate, to include: BH/ID team meetings, stakeholder's meetings, case conferences and community meetings

22. As assigned, schedule necessary medical and dental appointments for residents

23. Ensure all required documentation is completed in a timely, comprehensive and legible manner and in accordance with PATH policies and procedures

24. Must have regular access to an electronic device for periodic business communication and employee self-service capabilities. This may be a smartphone, tablet, Chromebook, laptop, or home computer. Business communications would include daily checking of email and submitting leave time requests but would not be needed during all hours of employment.

Qualifications:

2 years' experience in human services, paid or unpaid, preferably with adults with psychiatric and/or intellectual disabilities; able to use Electronic Health Record.
